As your treatment plan nears completion, your care team will begin preparing you for discharge. You will receive instructions regarding medications, follow-up appointments, and any necessary lifestyle or activity adjustments. It is important to understand your care plan at home, including when to seek medical attention if needed.

Please ensure all personal belongings are collected and any administrative requirements are completed prior to leaving. Our team is available to answer your questions and support a smooth and safe transition from hospital to home.
